What Is Plantar Plate Repair?

Plantar plate repair is a surgical procedure performed to treat injuries or tears of the plantar plate in the foot. The plantar plate is a thick band of fibrous tissue located on the plantar (bottom) surface of the metatarsophalangeal (MTP) joints, which are the joints connecting the metatarsal bones (foot bones) to the proximal phalanx bones (toe bones). This procedure helps stabilize the MTP joints and prevent the toes from deviating or dislocating.

Injuries to the plantar plate can occur due to trauma, overuse, or underlying foot conditions, such as bunions or hammertoes. When the plantar plate is damaged, it can lead to pain, inflammation, instability of the MTP joint, and deformities of the toes.

Procedure 

During plantar plate repair surgery, the patient typically undergoes local or regional anesthesia to numb the foot. The surgeon then incises over the affected MTP joint to access the damaged plantar plate. After carefully examining the extent of the plantar plate injury, the surgeon repairs and reinforces the damaged tissue using sutures or specialized anchors. In some cases, the surgeon may need to adjust surrounding soft tissues, tendons, or ligaments to achieve proper alignment and stability of the MTP joint. Upon completion of the plantar plate repair and any necessary soft tissue adjustments, the surgeon closes the incision with sutures and dresses and bandages the surgical site.

Rehabilitation and Therapy

Following surgery, the patient may require crutches or a walker to maintain non-weight-bearing on the foot, promoting proper healing. Additionally, a cast or specialized boot may shield the foot during the initial recovery phase.

Recovering from plantar plate repair entails rest, elevation, and gradually resuming weight-bearing activities under a foot and ankle specialist’s guidance. Physical therapy and rehabilitation exercises may be prescribed to facilitate recovery and restore foot function and mobility.

Plantar plate repair is generally effective for treating injuries, addressing pain, stabilizing the MTP joint, and preventing toe deformities. Nonetheless, potential risks and complications, such as infection and delayed wound healing, accompany surgical procedures.
