What is Patellofemoral Pain Syndrome?

Patellofemoral pain syndrome, or runner’s knee, describes a common knee condition that develops when irritation affects the cartilage, soft tissues, or other structures surrounding the joint between the kneecap (patella) and the thighbone (femur). Repeated stress, poor kneecap tracking, muscle imbalances, or excessive pressure within the patellofemoral joint can trigger inflammation and pain without causing damage to the bones themselves. The condition most often produces a dull, aching pain around or behind the kneecap that becomes more noticeable during activities that place greater force on the front of the knee.

Patellofemoral pain syndrome affects the way the kneecap moves within the groove at the end of the thighbone during bending and straightening of the knee. When the kneecap fails to glide smoothly, increased friction and pressure place extra strain on the surrounding tissues, leading to pain and reduced function. Although the condition does not involve a fracture or ligament tear, ongoing irritation can limit physical activity and make everyday movements more difficult. Early diagnosis and appropriate treatment help restore proper knee mechanics, reduce pain, and improve long-term joint function.

Symptoms of Patellofemoral Pain Syndrome

Symptoms may include:

  • Dull, aching pain around or behind the kneecap

  • Increased pain during running, jumping, or squatting

  • Discomfort while climbing or descending stairs

  • Pain after sitting with bent knees for extended periods

  • Grinding, popping, or clicking sensations during knee movement

  • Knee stiffness after prolonged sitting or rest

  • Tenderness around the kneecap

  • Mild swelling around the front of the knee

  • Reduced knee strength during physical activity

Causes of Patellofemoral Pain Syndrome

Patellofemoral pain syndrome develops when stress, pressure, or movement changes affect the way the kneecap moves within the knee joint. Muscle weakness, poor alignment, overuse, and repeated strain can place extra stress on the tissues around the kneecap and trigger pain.

Causes may include:

  • Repetitive activities such as running, jumping, or squatting

  • Sudden increases in exercise intensity or training volume

  • Weak thigh, hip, or core muscles

  • Tight muscles around the hips, thighs, or knees

  • Poor kneecap tracking during movement

  • Improper movement patterns during physical activity

  • Foot alignment problems that affect knee mechanics

  • Muscle imbalances that change knee stability

  • Direct impact or injury to the front of the knee

  • Prolonged activities that place pressure on the kneecap

Treatment Options for Patellofemoral Pain Syndrome

Treatment for patellofemoral pain syndrome depends on symptom severity, activity level, and the underlying cause. Mild cases often improve with conservative care that reduces irritation, strengthens supporting muscles, and improves movement patterns. Severe cases may require more advanced treatment when pain continues, affects daily activities, or does not improve with initial approaches.

Mild cases:
Mild symptoms often respond to activity modification, ice therapy, and over-the-counter pain relievers when recommended. Stretching tight muscles and strengthening the quadriceps, hips, and core can improve knee support and reduce stress on the kneecap. Supportive footwear, orthotics, and improved exercise techniques may also help correct movement problems and prevent recurring pain.

Severe cases:
Severe symptoms may require a structured physical therapy program that focuses on improving knee stability, muscle strength, and kneecap movement. Specialists may recommend braces, taping techniques, or additional medical treatments when appropriate. Rare cases that continue despite long-term conservative care may require surgical evaluation to address underlying structural problems. A customized rehabilitation plan helps restore function and support a safe return to activity.
