COVID-19 POLICY

Employees:

• Law- mandated vaccinations of all health care workers. Must provide proof through vaccination records. 
• Employees shall follow the COVID-19 government quarantine mandate if symptomatic. 

Patients:

• Patients: Staff/front desk will ask whether patients have any COVID like symptoms in the past week. If symptomatic, patients must wear a mask in clinic, supplied to patient if necessary. 

Patients and Employees: 

• Staff and physicians will wear masks upon patient request.
